Description
Centered within the bony orbit, the globe sits anterior to the orbital apex with the sclera and blue iris visible at the front of the eye. Four rectus muscles, superior, inferior, medial, and lateral rectus, course from a posterior common tendinous origin near the optic canal to insert on the sclera anterior to the equator, forming the familiar cone around the optic nerve as it exits posteriorly. The superior oblique runs along the superomedial orbit toward its trochlea and reflects posterolaterally across the superior aspect of the globe, while the inferior oblique arises anteromedially from the orbital floor and sweeps laterally beneath the globe to its posterior scleral insertion. Orbital walls are sectioned to expose the relationship of muscle bellies and tendons to the confines of the socket. Orbital musculature is where anatomy meets function, and small spatial errors change the story. The close proximity of the medial rectus to the lamina papyracea and ethmoidal air cells explains why medial orbital wall fractures and endoscopic sinus surgery can present with new-onset diplopia, while lateral rectus involvement raises immediate concern for orbital apex pathology. Oculomotor coordination is anchored at the annulus of Zinn, so a sectional view that preserves the muscle cone and optic nerve helps teach why lesions at the superior orbital fissure can couple ophthalmoplegia with sensory deficits.
