Description
Shown in profile, the adult male globe sits centrally with the cornea anterior to the iris and pupil, while the extraocular muscles sleeve the sclera posterior to the limbus. The superior, inferior, medial, and lateral rectus muscles arise from the common tendinous ring at the orbital apex and run anteriorly in a straight course to insert on the anterior half of the sclera, with the medial rectus positioned nasally and the lateral rectus temporally. Superior oblique originates superomedially, passes anteriorly toward the trochlea, then reflects posterolaterally to insert on the posterosuperior sclera, while inferior oblique arises anteromedially from the orbital floor and courses posterolaterally beneath the globe to the posteroinferior sclera. Tendinous insertions and surrounding fascial planes are indicated around the muscle bellies. Extraocular muscle topography matters because ocular motility findings map directly onto individual muscle actions and their cranial nerve supply, oculomotor (CN III) to most muscles, trochlear (CN IV) to superior oblique, and abducens (CN VI) to lateral rectus. This arrangement also explains classic patterns such as a CN IV palsy producing hypertropia with excyclotorsion and compensatory head tilt, or thyroid eye disease restricting elevation in abduction from inferior rectus fibrosis. Strabismus surgery depends on these insertional relationships, including the spiral of Tillaux and the proximity of the superior oblique tendon to the superior rectus.
