Description
Anterior orbital anatomy is organized around the globe, with the superior rectus and inferior rectus muscles running straight in the sagittal plane from the posterior orbit toward their insertions on the superior and inferior sclera, just posterior to the corneoscleral limbus. Medially and laterally, the medial rectus and lateral rectus would be expected to flank the globe and insert on the anterior half of the sclera, while the superior oblique tendon typically courses anteromedially to the trochlea before turning posterolaterally beneath the superior rectus to reach the posterolateral globe. Small but instructive landmarks sit adjacent to these muscle paths, including Tenon’s capsule enveloping the globe and the conjunctival reflection near the rectus insertions. Rectus and oblique orientation matters because motility deficits localize to specific muscles and cranial nerves with a precision you do not get elsewhere in head and neck anatomy. Third nerve palsy classically weakens the superior, inferior, and medial rectus and the inferior oblique, leaving unopposed lateral rectus (CN VI) and superior oblique (CN IV) action, a pattern that explains the characteristic abducted, depressed resting eye position and diplopia in clinical exam. Strabismus surgery also depends on these anterior relationships: surgeons identify rectus insertions at predictable millimeter distances from the limbus and plan recession or resection while protecting adjacent structures like the anterior ciliary vessels that travel with the rectus muscles.
