Seen from a superior aspect, the liver’s diaphragmatic surface forms a broad convex dome, with the right lobe extending laterally and posteriorly beyond the smaller left lobe. The falciform ligament typically marks the sagittal plane division between lobes along the anterosuperior surface, while the rounded superior margin reflects its close apposition to the central tendon of the diaphragm. Posteriorly, the bare area and the groove for the inferior vena cava lie near the superoposterior aspect of the right lobe, deep to the diaphragm. A superior view matters because it matches how the liver is encountered against the diaphragm on cross sectional imaging and during approaches that mobilize the organ from above. Diaphragmatic attachments, including the coronary and triangular ligaments, define the safe planes for mobilization in hepatectomy and trauma exposure, and they explain why subphrenic collections and right pleural effusions can accompany hepatic pathology. Orientation to the dome also helps when teaching the hepatic venous outflow pattern, where the right, middle, and left hepatic veins course superiorly to drain into the inferior vena cava just beneath the diaphragm.
