Description
Viewed from superior, the diaphragmatic surface of the male liver is opened to reveal the intrahepatic venous tree draining toward the caval confluence. Major hepatic veins (right, middle, and left) course centrally within the parenchyma and converge superiorly and posteriorly to enter the inferior vena cava in the retrohepatic groove, immediately inferior to the diaphragm’s central tendon. Smaller venules radiate from peripheral segments toward these larger trunks, outlining venous territories across the right and left anatomical lobes. A superior perspective puts the hepatocaval junction and short extrahepatic stumps of the hepatic veins in one frame, which is exactly where bleeding becomes difficult to control during mobilization of the liver. Surgeons think here during hepatic resection planning and liver transplantation, where variations in the middle and left hepatic veins, accessory inferior right hepatic veins, and proximity to the vena cava affect clamp placement and the feasibility of venous reconstruction. Budd-Chiari syndrome and hepatic venous outflow obstruction also make more sense when you can trace drainage from peripheral parenchyma to the obstructed outflow near the diaphragm. Small details matter. Use this artwork in gross anatomy and hepatobiliary teaching to distinguish hepatic venous drainage to the inferior vena cava from the portal venous inflow system, and in surgical atlases or journal figures describing right hepatectomy, extended left hepatectomy, or piggyback transplant technique with caval preservation.
