Description
Oriented in posterior aspect, the liver is presented with the diaphragmatic surface facing the viewer and the bare area (area nuda hepatis) exposed where peritoneum is absent and hepatic parenchyma lies directly against the diaphragm. Superiorly and to the right, the convex right lobe dominates; the left lobe extends superomedially with a sharper margin. Along the posterior surface, the groove for the inferior vena cava runs vertically, with the caudate lobe positioned between the IVC groove and the fissure for the ligamentum venosum. Landmarks matter. Posterior liver anatomy is where surface description becomes operative anatomy. The bare area and its peritoneal reflections frame the coronary ligament and triangular ligaments, key reference points when mobilizing the right lobe during open or laparoscopic hepatectomy, trauma exploration, or subdiaphragmatic abscess drainage. The relationship to the retrohepatic inferior vena cava is unforgiving, and this view supports teaching why uncontrolled bleeding can occur when short hepatic veins are avulsed during liver mobilization. Use this rendering to anchor gross anatomy lab teaching of peritoneal reflections, to illustrate surgical atlas chapters on right hepatic mobilization and retrohepatic IVC exposure, or to support radiology correlates when explaining posterior diaphragmatic impressions and the bare area on cross-sectional imaging. It also fits well in hepatology education when introducing segmental concepts with external landmarks before moving to Couinaud segmentation.
