Description
Viewed from the superior aspect, the occipital bone is presented from its endocranial surface, centered on the foramen magnum and the surrounding basilar, lateral, and squamous parts. Radiating from the internal occipital protuberance, the cruciform eminence divides the occipital squama into fossae that accommodate the occipital lobes superiorly and the cerebellar hemispheres inferiorly, while pronounced grooves track laterally and superiorly for the dural venous sinuses. The sagittal sulcus continues toward the groove for the superior sagittal sinus, meeting the transverse sinus grooves at the confluence, and a midline sulcus descends toward the internal opening of the foramen magnum for the occipital sinus. Attention to these sinus grooves matters because their depth and asymmetry influence both normal venous drainage patterns and operative planning around the posterior cranial fossa. Occipital fractures can cross the transverse or sigmoid sinus grooves and produce dural venous sinus thrombosis or hemorrhage, a scenario that changes how you interpret head CT and when you escalate to CT venography. For a posterior fossa craniotomy, the internal occipital protuberance and transverse sinus grooves set safe limits for burr-hole placement and bone removal. Small errors here bleed.
