Description
Viewed from above into the posterior cranial fossa, the superior sagittal sinus runs in the midline along the attached margin of the falx cerebri and expands posteriorly into the confluence of sinuses (torcular Herophili). The straight sinus approaches the confluence from an anteroinferior direction at the junction of falx cerebri and tentorium cerebelli, while the right and left transverse sinuses sweep laterally from the confluence along the posterolateral attachment of the tentorium. Smaller superficial cerebral veins and bridging veins converge toward the superior sagittal sinus and adjacent dural channels. Bone of the cranial base and posterior skull provides spatial context for the dural reflections and nearby foramina. This superior perspective matters because the torcula is a common crossroads for venous flow patterns and an anatomic reason why posterior fossa and falcine lesions can produce variable venous hypertension depending on transverse sinus dominance. It is also the region where dural arteriovenous fistulas frequently recruit cortical venous reflux, a high-risk configuration that neuroradiologists map carefully on CT venography, MR venography, and digital subtraction angiography. A small mismatch in understanding which sinus is continuous with the straight sinus or which transverse sinus is dominant can change how you interpret venous sinus thrombosis, especially when clot extends from the superior sagittal sinus into the transverse-sigmoid outflow.
