Description
Seen from a superior (dorsal) perspective, the paired cerebral hemispheres flank the midline superior sagittal sinus, which runs anteroposteriorly within the attached margin of the falx cerebri. Along either side of this dural venous sinus, multiple venous lacunae occupy the parasagittal dura mater, forming irregular, often asymmetric spaces that communicate with the superior sagittal sinus and overlie the medial convexity of the cerebral cortex. Superficial cerebral veins approach these parasagittal channels from the superolateral surface, crossing the cortical gyri and sulci before draining toward the midline. Dura first, then cortex. For teaching and clinical correlation, this top-down view clarifies why parasagittal venous anatomy matters in the operating room and on imaging: the venous lacunae can be prominent reservoirs adjacent to the superior sagittal sinus and may receive bridging veins that are vulnerable during interhemispheric or parasagittal approaches. Their variable size and laterality also explain occasional asymmetry on contrast-enhanced MR venography or CT venography, where lacunae can mimic focal sinus irregularity if you do not track their continuity with the sinus and adjacent cortical veins. Extra-axial collections and trauma enter the differential here, because tearing of parasagittal bridging veins near the sinus and lacunae is a common mechanism for subdural hematoma and can complicate dural opening close to the midline.
