Description
Oriented in superior view, the hyoid appears as a U or horseshoe shaped element with a central corpus (body) spanning the midline and paired cornua projecting posterolaterally. The greater horns extend laterally and slightly posterior from the body, while the lesser horns arise at the junction of body and greater horn as small superiorly directed spurs, positioned closer to the midline. Adjacent laryngeal framework may be appreciated deep to the hyoid, with the thyroid cartilage lying inferior and posterior relative to the hyoid body and the thyrohyoid membrane bridging the interval. Seen from above, the hyoid is the fixed bony anchor for the suprahyoid and infrahyoid musculature, so this perspective clarifies why small shifts in its position translate into changes in airway caliber and laryngeal elevation during swallowing. For operative anatomy, the greater cornua define the lateral limits of dissection in approaches to the vallecula, epiglottis, and pre-epiglottic space, and they remain a key landmark when orienting to the superior laryngeal neurovascular bundle traversing near the thyrohyoid membrane. Fracture of the hyoid, most often at the greater horn, is also a classic forensic and clinical finding in manual strangulation and high-energy anterior neck trauma. A small bone, big implications.
