Description
Bronchial anatomy is presented with emphasis on the right middle lobar bronchus (bronchus lobaris medius) and its segmental outflow that aerates the middle lobe of the right lung. The bronchus intermedius continues inferior to the right superior lobar bronchus and gives rise to the middle lobar bronchus on its anterolateral aspect, before continuing distally to the right inferior lobar bronchus. From the middle lobar bronchus, segmental bronchi to the medial and lateral segments course peripherally toward their respective bronchopulmonary segments. This airway matters because the right middle lobe sits between the horizontal and oblique fissures, and its bronchus is relatively long and narrow with a dependent takeoff, a combination that contributes to the classic tendency for right middle lobe atelectasis and the so-called middle lobe syndrome in the setting of mucus plugging, asthma, or extrinsic compression from enlarged hilar lymph nodes. Bronchoscopy relies on these branching relationships: you identify the bronchus intermedius first, then turn anteriorly into the middle lobar bronchus to suction secretions or sample a localized infection. A frequent teaching point. The right middle lobe is also a common site of recurrent nontuberculous mycobacterial infection in older adults, where targeted lavage and cultures depend on accurate segmental navigation. Pulmonology and thoracic surgery teams can use this artwork for bronchoscopy training, operative planning for middle lobectomy, and correlation with CT airway anatomy in radiology teaching files.
