Description
Centered on the right pulmonary hilum, the middle lobar vein is traced as a blue venous channel draining the right middle lobe and coursing medially toward the left atrium. It travels in close company with the right middle lobar bronchus, and its tributaries run anterior and inferior to the more superior venous drainage from the right upper lobe. Proximally, the vessel converges with adjacent pulmonary veins to form the right superior pulmonary vein before entering the posterior aspect of the left atrium. Clear color coding separates the pulmonary veins from nearby pulmonary arterial branches and the ascending aorta. Middle lobe venous anatomy matters whenever the right hilum is approached surgically, because small variations in venous confluence can place the middle lobar vein at risk during right upper lobectomy or middle lobectomy. A common pitfall is mistaking a middle lobe venous tributary for an upper lobe vein, which can lead to venous congestion of the preserved lobe postoperatively. The adjacent trachea and main bronchi provide reliable airway landmarks for orienting hilar dissection and for correlating with CT angiography and bronchoscopy-based teaching.
