Description
Running along the lateral aspect of the right lung, a pulmonary venous tributary courses medially toward the hilum and joins the right pulmonary venous drainage entering the left atrium. From an anterior perspective, the vessel sits anterior to the right main bronchus at the hilum, while the right pulmonary artery occupies a more superior and posterior position relative to the pulmonary veins. Superior and inferior pulmonary veins are seen converging toward the cardiac base, with the trachea superiorly and the great vessels, including the ascending aorta and pulmonary trunk, framing the central mediastinum. Clear separation of bronchus, pulmonary artery, and pulmonary veins is the teaching point in this orientation, because it mirrors what you confront in right-sided hilar dissection and VATS lobectomy. Misidentification of a pulmonary vein branch can lead to venous congestion of the remaining lobe or troublesome hemorrhage, and an anterior view helps reinforce that pulmonary veins generally lie anterior and inferior to the pulmonary artery at the hilum. Correlating the lateral right pulmonary venous pathway to left atrial inflow also supports interpretation of contrast-enhanced CT and pulmonary venous phase angiography.
