Description
Anterior thoracic anatomy is rendered with the heart centered and the left lung hilum opened to emphasize the superior basal vein (vena basalis superior) draining the basal segments of the left lower lobe. From the lower lobe parenchyma, this blue venous channel courses superiorly and medially toward the left inferior pulmonary vein, lying anterior to the corresponding segmental bronchus and closely related to branches of the left pulmonary artery at the hilum. Superiorly, the trachea bifurcates into the main bronchi, and the pulmonary veins converge on the left atrium while the ascending aorta and pulmonary trunk frame the mediastinum. This venous relationship matters when you need to teach or plan around left lower lobectomy and segmentectomy, where the venous anatomy often dictates the safe parenchymal division line and helps prevent inadvertent congestion of preserved segments. Lymph node dissection around the inferior pulmonary vein and bronchus (stations 9 and 10) also benefits from a clear mental model of which venous tributaries can be sacrificed versus which should be preserved. Small variations are common, including separate venous drainage from the superior segment (S6) into the left atrium or shared trunks with other basal veins, a frequent source of intraoperative bleeding when misidentified.
