Description

Arising from the inferior lobe of the left lung, the superior basal vein (vena basalis superior) collects venous blood from the superior segment of the lower lobe and runs centrally toward the left hilum. From an anterior thoracic perspective with the heart centered, the vein appears as a blue pulmonary venous tributary coursing superiorly and medially through the peribronchovascular connective tissue, lying in close relationship to the segmental bronchus and accompanying pulmonary arterial branches. At the hilum it converges with adjacent basal venous channels to form the inferior pulmonary vein before entering the left atrium. Segmental bronchi and bronchioles arborize peripherally around this venous pathway. Recognizing this specific venous tributary matters when you need to correlate segmental anatomy with surgical planes in the left lower lobe. During a left lower lobectomy or a basal segmentectomy (S6 versus basal segments), the superior basal vein can be mistaken for neighboring basal venous branches if you do not track it back to the inferior pulmonary vein, and that error can compromise venous drainage of a preserved segment. Preoperative CT angiography and intraoperative hilar dissection both rely on a clear mental map of how bronchi, pulmonary arteries, and pulmonary veins interdigitate, and this illustration makes that relationship explicit.
