Description
Arising from the apicoposterior segment of the left upper lobe, the left apicoposterior vein courses inferiorly and medially toward the left superior pulmonary vein, running in close relationship to the segmental bronchus and accompanying pulmonary arterial branches. Proximally, it converges with adjacent upper-lobe venous tributaries near the left hilum, where the pulmonary veins lie anterior and inferior to the main bronchus in the root of the lung. In the same field, the left atrium receives the pulmonary venous return posterior to the great arterial outflow tracts, while the trachea bifurcates superiorly into the main bronchi. Small but specific. Segmental pulmonary venous anatomy matters in the operating room and on cross sectional imaging because venous variants can change the safe plane of dissection during left upper lobectomy or segmentectomy of S1 plus S2. A prominent apicoposterior vein may drain separately into the left atrium or join the anterior segmental vein, and mistaking it for an arterial branch at the hilum can lead to troublesome hemorrhage or venous congestion of the preserved segments. This perspective also supports teaching the classic anterior to posterior hilar arrangement on the left, a recurring source of orientation errors during VATS and robotic approaches. Use this illustration in thoracic surgery atlases to accompany discussions of left upper-lobe segmentectomy, and in radiology teaching files when correlating CTA or contrast enhanced CT with pulmonary venous drainage patterns. It also fits cardiopulmonary anatomy modules that integrate the left atrium, pulmonary veins, and bronchial tree rather than treating them as isolated systems.
