Description
Centered in the thorax, the heart sits between the right and left lungs, with the trachea descending superiorly before bifurcating into the main bronchi. Within the right lung, the posterior pulmonary vein is traced in blue along the posterior aspect of the hilum, collecting venous tributaries from the posterior segments and converging toward the right pulmonary venous ostium at the left atrium. Red pulmonary arteries accompany the bronchial tree more closely, while the venous channels run more intersegmentally, crossing behind or between bronchopulmonary segments as they course medially. Spatially, the highlighted vessel lies posterior to the right main bronchus at the root of the lung, then turns medially toward the heart. Emphasis on the posterior vein of the right lung matters when you are teaching pulmonary segmental anatomy and the practical difference between pulmonary arterial and venous patterns. Segmentectomy planning and video-assisted thoracoscopic surgery depend on identifying veins that mark intersegmental planes, and on avoiding inadvertent ligation that can congest a preserved segment. Misidentification at the hilum is a real problem. The posterior venous tributaries can also be relevant in right-sided pulmonary venous isolation for atrial fibrillation, where anatomic variation and ostial orientation affect catheter access and ablation strategy.
