Description
Seen from the right lateral aspect, the right main bronchus continues as the bronchus intermedius while the right upper lobar bronchus (eparterial) branches superiorly toward the upper lobe. From that lobar bronchus, the anterior segmental bronchus (B3) courses anterolaterally toward the anterior segment (S3), positioned anterior to the posterior segmental branch (B2) and typically inferior to the apical segmental bronchus (B1). Distally, the segmental bronchus tapers as it approaches its segmental distribution within the upper lobe parenchyma. Orientation is anchored to the hilum, with proximal airway caliber larger and cartilaginous, and distal branching progressively smaller. A right lateral airway view is the one you want when teaching the eparterial anatomy that distinguishes the right upper lobar bronchus and when mapping segmental targets for bronchoscopy. B3 is a frequent pathway for endobronchial sampling in suspected anterior upper-lobe disease, and it is an airway surgeons keep straight when planning segmentectomy of S3 or when localizing an endobronchial lesion relative to the segmental carinae. Misidentifying B3 as a B1 or B2 branch is an easy error when relying on frontal projections alone. Side-on geometry clarifies the anterior takeoff.
