Description
Seen in lateral profile, the superficial trapezius spans from the occipital region and nuchal ligament down to the cervical and upper thoracic spinous processes, then fans laterally toward the clavicle, acromion, and spine of the scapula. Superior fibers angle inferolaterally toward the lateral third of the clavicle, middle fibers run more horizontally to the acromion and scapular spine, and inferior fibers course superolaterally toward the medial end of the scapular spine. Along its anterior border, the sternocleidomastoid descends to the manubrium and clavicle, while the masseter and temporalis occupy the lateral face and temporal fossa superiorly. Serratus anterior digitations and the lateral thoracic cage sit inferior and anterior to the scapula, with the scapular body positioned posterolateral to the ribs. A lateral trapezius view is the cleanest way to teach scapulothoracic mechanics because you can follow fiber direction directly into function, elevation and upward rotation by upper fibers, retraction by middle fibers, and depression with upward rotation by lower fibers acting with serratus anterior. Clinically, that division matters during assessment of spinal accessory nerve (CN XI) injury after posterior triangle lymph node biopsy or neck dissection, where trapezius weakness produces lateral scapular winging and difficulty initiating abduction above 90 degrees. Palpation landmarks are obvious. The clavicle, acromion, and scapular spine frame the muscle’s insertions and guide safe injection and incision planning.
