Description
Posterior surface anatomy of the female upper back centers on the trapezius, highlighted as a broad, superficial sheet spanning from the external occipital protuberance and nuchal ligament inferiorly to the spinous processes of the upper thoracic vertebrae, then laterally to the lateral third of the clavicle, acromion, and spine of the scapula. Superior fibers descend to the shoulder girdle, middle fibers run transversely toward the scapular spine, and inferior fibers ascend toward the medial end of the scapular spine, forming the familiar triangular contour over the cervicothoracic junction. Laterally, the posterior deltoid caps the shoulder, while the infraspinatus and teres minor occupy the infraspinous fossa with teres major coursing toward the humerus; inferior to the trapezius margin, latissimus dorsi broadens across the lower thoracolumbar back. Clear landmarks. This posterior view matters because trapezius function is judged by scapular position and motion, not by isolated muscle bulk. Injury to the spinal accessory nerve (CN XI), often encountered during posterior triangle lymph node biopsy or neck dissection, produces shoulder droop, impaired scapular upward rotation during abduction, and lateral scapular winging that contrasts with the medial winging of serratus anterior palsy. The trapezius also guides surface palpation for trigger points along the upper fibers, a common pain generator in cervical myofascial syndromes, and it frames the midline where thoracic spinous processes and scapular spine level are taught.
