Description
Centered on the upper thoracic back, the rhomboideus minor and rhomboideus major span obliquely from the midline spinous processes to the medial (vertebral) border of the scapula. Rhomboideus minor lies superior to rhomboideus major, typically attaching near the root of the scapular spine, while rhomboideus major extends inferiorly toward the inferior angle. Superficial fibers of trapezius overlie the rhomboids, and deeper to them sit serratus anterior on the costal surface of the scapula and the posterior thoracic wall. Clear fiber direction makes the scapulothoracic relationship easy to read. Posterior orientation matters here because rhomboid function is inseparable from scapular positioning: they retract and downwardly rotate the scapula while helping hold the medial border flush against the thoracic cage during pulling and overhead tasks. A short muscle. A common clinical correlation is dorsal scapular nerve neuropathy (C5), which can produce medial scapular pain, weakness of retraction, and subtle lateral drift of the scapula that is often mistaken for primary rotator cuff pathology. This view also helps distinguish rhomboid strain from trapezius or levator scapulae pain patterns, and it clarifies why scapular dyskinesis can persist when the glenohumeral exam is otherwise normal. Useful for teaching posterior shoulder girdle anatomy in gross lab and kinesiology courses, and for illustrating scapular stabilizer assessment in physical therapy, sports medicine, and orthopedics content aimed at posture-related upper back pain.
