Description
Centered on the male anterior shoulder and upper thorax, the anterior circumflex humeral vein courses laterally and posteriorly around the surgical neck of the humerus, running deep to the deltoid as it approaches the quadrangular space region. Proximally it communicates with venous channels accompanying the anterior circumflex humeral artery, then drains medially toward the axillary vein in the infraclavicular fossa. Surface landmarks such as the nipple and anterior axillary fold help orient the vessel’s position relative to pectoralis major and the deltopectoral interval. A small vessel with outsized teaching value. Relationship anatomy in this region comes up whenever you work near the proximal humerus. Fractures at the surgical neck and operative fixation can disrupt circumflex humeral vessels, and although the posterior circumflex system is often emphasized, the anterior circumflex humeral vein and its accompanying artery mark the anterior periosteal corridor where retraction and deltoid splitting can lead to troublesome bleeding. This anterior perspective also clarifies why venous drainage from the deltoid and humeral head region converges quickly on the axillary vein, a point that matters when planning incisions and hemostasis for open reduction and internal fixation or shoulder arthroplasty.
