Running posterior to the surgical neck of the humerus, the posterior circumflex humeral vein curves laterally beneath the deltoid and tracks with the posterior circumflex humeral artery through the quadrangular space. Proximal to its circumferential course, the vessel lies deep to the posterior fibers of deltoid and superficial to the long head of triceps, with the axillary nerve accompanying it as it passes inferior to teres minor and superior to teres major. Medially, the venous return converges toward the axillary vein in the axilla, while more distal tributaries follow the posterior arm toward the brachial venous system. Spatial relationships read cleanly in this posterior shoulder and upper arm perspective. Clinically, this venous corridor matters because it occupies the same confined interval that places the axillary nerve at risk in fractures of the proximal humerus and posterior shoulder dislocations, and venous injury here can add swelling and hematoma that obscure neurovascular assessment. The circumflex vessels also sit in the operative field for deltoid splitting approaches and posterior exposures around the quadrangular space, where misdirected dissection can compromise deltoid perfusion and produce brisk bleeding.
