Description
Anteriorly, the liver is rendered with a semi-transparent parenchyma so the major inflow vessels and biliary tree can be followed in situ. The portal vein approaches the porta hepatis from inferior and posterior, where it lies posterior to the proper hepatic artery and adjacent to the common hepatic duct in the hepatoduodenal ligament. Green-highlighted right and left hepatic ducts emerge from the hepatic hilum superior to the neck of the gallbladder, while the cystic duct descends to join the common hepatic duct and form the common bile duct running inferiorly toward the duodenum. Clear separation of vascular inflow from bile drainage matters when teaching the porta hepatis and Calot’s triangle, where the cystic artery commonly branches from the right hepatic artery and traverses close to the cystic duct. This anterior perspective supports clinical discussions of obstructive jaundice from choledocholithiasis, iatrogenic bile duct injury during laparoscopic cholecystectomy, and portal hypertension where altered portal venous flow and portosystemic collateral formation become relevant. Small structure, big consequences.
