Description
Framed from an anterior perspective, the lower lumbar vertebrae transition into the sacrum and coccyx, with the coccygeal plexus represented as fine nerve twigs emerging inferior to the sacral plexus and clustering along the anterior surface of the coccyx. The ventral rami of S4, S5, and the coccygeal nerve (Co1) converge medial to the pelvic bones, then course inferiorly toward the pelvic floor and perineal region. Bilateral iliac bones border the field laterally, while the lumbosacral trunk and more superior lumbar plexus elements appear proximal and superior to the coccygeal network. Small ganglion-like nodal points may be indicated near the midline, consistent with pelvic autonomic communications in this region. An anterior rendering of the coccygeal plexus matters because it aligns with the operative and clinical corridor used when evaluating coccydynia, perineal pain, and distal sacral radiculopathy, where symptoms can be referred to the anococcygeal area via the anococcygeal nerves. The relationship of these fibers to the pelvic floor explains why pain can worsen with sitting, childbirth-related pelvic floor injury, or direct trauma to the tailbone. It also supports teaching around caudal epidural approaches and why low sacral segments may evade more cranial blocks.
