Description
Centered on the descending thoracic aorta as it tracks inferiorly along the anterior surface of the vertebral bodies, this composition resolves the aortic plexus as a dense mesh of autonomic filaments and small ganglionic enlargements clustered around the arterial adventitia. Sympathetic trunks run longitudinally on the anterolateral margins of the vertebral column, with thoracic splanchnic nerves coursing medially toward the prevertebral region and contributing fibers to the peri-aortic network near arterial branch points. Posteriorly, the thoracic vertebrae and intervertebral discs establish depth, while ribs arc laterally from the costovertebral joints to frame the mediastinal corridor. Blue lymphatic channels and nodal stations parallel the great vessel, reinforcing the close packing of neural, vascular, and lymphatic structures in the posterior mediastinum and upper retroperitoneum. For teaching autonomic anatomy, the peri-aortic plexus is where abstract pathways become clinically concrete: sympathetic and parasympathetic fibers converge around the aorta and redistribute along its branches to reach foregut, midgut, and hindgut targets. That anatomy matters during thoracoabdominal aortic repair and retrocrural exposure, when traction, cautery, or dissection near the aorta can injure prevertebral autonomic fibers and contribute to postoperative ileus, altered visceral pain patterns, or, at lower levels, ejaculatory dysfunction from disruption of sympathetic outflow.
