Description
Arising from the aortic plexus on the anterior surface of the abdominal aorta, slender ureteric branches course inferolaterally toward the retroperitoneal ureter, crossing the anterior aspect of the lumbar vertebral bodies before descending toward the pelvic brim. Superiorly, lower ribs and the upper lumbar spine frame the field, while the ilium, ischium, and pubis form the bony boundaries of the pelvis inferiorly and laterally. The nerve network is rendered in bright yellow against off-white bone, clarifying the relationship between the periarterial autonomic fibers and the ureter’s longitudinal path from abdomen to pelvis. Spatial orientation is straightforward. Midline spine, bilateral pelvic ring. Autonomic innervation to the ureter is clinically relevant because ureteric colic pain patterns track along sympathetic pathways rather than the ureter itself, classically referring from the flank toward the groin as the stone migrates distally. An anterior perspective on the aortic plexus helps explain why retroperitoneal surgery near the abdominal aorta and common iliac vessels, including lymph node dissection, aortoiliac reconstruction, or anterior spinal exposure, can disturb periureteric autonomic fibers and contribute to dysmotility, postoperative ileus overlap, or altered visceral pain signaling. For trainees, this is also a clean way to separate the aortic plexus contributions from the somatic nerves that dominate the lumbosacral plexus in the same region. Suitable for gross anatomy and neuroanatomy teaching on autonomic plexuses, urology modules covering ureteral innervation and referred pain, and surgical atlas figures for anterior retroperitoneal approaches where ureter identification and protection are emphasized.
