Description
Running anterior to the lumbar vertebral bodies, the abdominal aorta descends in the midline and bifurcates inferiorly into the right and left common iliac arteries at approximately the L4 level, with the inferior vena cava and adjacent iliac veins positioned slightly to the right and posterior. Draped over the aortic wall is the aortic plexus, an interlacing autonomic network formed by preganglionic and postganglionic sympathetic fibers and parasympathetic contributions, with small ganglia and nerve junctions clustered around the aortic bifurcation. Fine yellow nerve trunks course inferolaterally from the plexus to join the superior hypogastric plexus, while more proximal fibers relate to periarterial plexuses along major abdominal branches. Bony landmarks from the lower thoracic cage through the lumbar spine and the pelvic brim frame the field. An anterior perspective on the aortic plexus matters because it places autonomic fibers in the same working corridor used during transperitoneal exposure of the infrarenal aorta and aortoiliac bifurcation. Injury, traction, or thermal spread in this region can disrupt sympathetic pathways to the pelvis, a mechanism implicated in postoperative ejaculatory dysfunction after aortoiliac reconstruction or anterior lumbar approaches at L4 to S1. The relationship of the plexus to the aorta and common iliac origins also helps explain referred pain patterns and the autonomic component of pelvic vascular and visceral regulation. Small nerves.
