Description
Anteriorly oriented anatomy centers on the abdominal aorta descending on the anterior surface of the lumbar vertebral bodies before bifurcating at approximately L4 into the right and left common iliac arteries, with the iliac vessels tracking inferolaterally toward the pelvis. Draped over and alongside these major arteries, the abdominal autonomic plexuses form a yellow, reticular network, including peri-aortic fibers and the superior and inferior hypogastric plexuses as they continue into the pelvis. Proximal lumbar splanchnic contributions course medially from the lumbar sympathetic trunks, then redistribute around the great vessels toward the pelvic viscera. Midline, retroperitoneal topography is the organizing principle here. That vessel-centric arrangement matters clinically because surgeons often meet these plexuses in the same corridor used for para-aortic lymph node dissection, anterior lumbar spine exposure (ALIF), and aortoiliac procedures. Injury to the superior hypogastric plexus and hypogastric nerves during mobilization of the aortic bifurcation and iliac vessels is a well-described cause of ejaculatory dysfunction (including retrograde ejaculation) in male patients, and it can also contribute to postoperative pelvic autonomic symptoms. Fine fibers are easy to underestimate. This view keeps them honest. Use this illustration in gross anatomy teaching on retroperitoneal relationships, in autonomic nervous system modules covering lumbar splanchnic pathways and pelvic sympathetic distribution, and in surgical education materials for anterior approaches to the lumbosacral junction and aortoiliac reconstruction.
