Description
Centered on the abdominal aorta as it descends anterior to the lumbar vertebral bodies, the composition also includes the aortic bifurcation into the right and left common iliac arteries, continuing inferiorly toward their internal and external iliac branches. Yellow autonomic nerve trunks course parallel to the spine, consistent with the lumbar sympathetic trunks and their medial visceral branches forming the prevertebral abdominal plexuses along the aorta. Expect to see the periarterial nerve plexus investing the aorta and iliac vessels, with interconnections spanning the midline anterior to the vertebral column. Bone landmarks include lumbar vertebrae in the posterior field and the superior pelvis inferiorly, anchoring the neurovascular relationships in true anatomic position. For teaching autonomic anatomy, this is the cleanest way to connect named plexuses to real surgical territory: the aortic (intermesenteric) plexus and superior hypogastric plexus sit directly on the anterior aspect of the distal aorta and promontory, exactly where retroperitoneal dissection and vascular control occur. Injury or disruption to these sympathetic pathways during aortoiliac reconstruction, anterior lumbar interbody fusion at L4 to S1, or pelvic lymphadenectomy can contribute to ejaculatory dysfunction and bladder or bowel dysautonomia. The tight coupling of nerves to the great vessels also explains why sympathetic blockade and unintended neurapraxia can follow hematoma, aneurysmal dilation, or extensive nodal clearance around the aortic bifurcation.
