Description
Anterior neck dissection centers on the infrahyoid fascia (pretracheal layer of the deep cervical fascia) investing the strap muscles inferior to the hyoid bone. The sternohyoid and omohyoid lie superficially and more lateral, while sternothyroid and thyrohyoid sit deeper and more medial as they approach the thyroid cartilage; the fascia forms a midline raphe over the laryngeal framework. Laterally, the sternocleidomastoid borders the field, and inferiorly the fascia continues toward the manubrium and clavicles, blending with the fibrous planes over the thyroid gland and trachea. Blue emphasis in the anterior throat region logically corresponds to the laryngeal cartilages (thyroid and cricoid) and adjacent prelaryngeal soft tissue planes. Understanding this fascia matters because it defines the surgical and infectious compartments of the anterior cervical region. During thyroidectomy, tracheostomy, or cricothyrotomy, you work through these layers to separate midline strap muscles, expose the thyroid isthmus and tracheal rings, and control bleeding while respecting the anterior jugular venous system and the relationship of the strap muscles to the larynx. Deep neck space infections and postoperative hematoma often track along the pretracheal fascia into the superior mediastinum. Small planes, big consequences.
