Description
Anteriorly, the brachial fascia forms a continuous sleeve around the upper arm, blending superiorly with the fascia over the deltoid and pectoral region and tightening distally toward the epicondylar attachments. Beneath this investing layer, the anterior compartment lies anterior to the humeral shaft and intermuscular septa, enclosing the biceps brachii and brachialis, with the coracobrachialis positioned proximomedially along the medial humerus. Medially, the fascia thickens near the course of the basilic vein and medial cutaneous nerves of the arm, while laterally it approaches the cephalic vein tract and the lateral intermuscular septum that separates flexor from extensor compartments. Fascial compartment anatomy matters most when swelling has nowhere to go. In the upper arm, post-traumatic hematoma, anticoagulation-related bleeding, or a tight cast can increase intracompartmental pressure within the brachial fascia and septa, producing acute compartment syndrome with pain on passive elbow extension and evolving median or musculocutaneous nerve symptoms; delayed recognition risks ischemic necrosis of brachialis and biceps. Surgeons also use these fascial planes to plan incisions for anterolateral or medial approaches to the humerus, and to anticipate how infection or extravasated fluid tracks along intermuscular septa.
