Description
Posterior leg fascia is presented as a continuous connective tissue sleeve investing the crural musculature, enclosing the superficial and deep posterior compartments from the popliteal region toward the calcaneal tendon. Superficially, the sural fascia blends with the deep fascia of the leg (fascia cruris), while intermuscular septa extend anteriorly and laterally to meet the fibula and tibia, partitioning gastrocnemius and soleus from the deeper flexor mass. Medially and laterally, the fascial envelope relates to the posterior borders of the tibia and fibula, with the plantarflexor muscle bellies occupying the central posterior profile. Dense, fibrous tissue. Clear compartmental boundaries. Fascial anatomy in the posterior compartment matters when you are teaching why pressure elevation behaves differently across compartments and why a tense, noncompliant fascia cruris can convert edema or hemorrhage into a limb-threatening posterior compartment syndrome. For surgical planning, this gross view supports orientation for posterior compartment fasciotomy and helps explain where decompression must extend proximodistally to release the investing fascia over gastrocnemius and soleus and, when indicated, the deep posterior compartment beneath the soleal arch.
