Description
Arising from the inferior mesenteric artery, the left colic artery courses superiorly and to the left within the mesocolon, then divides into ascending and descending branches along the medial border of the descending colon. From an anterior perspective, the descending colon lies left lateral to the abdominal midline, with the sigmoid colon inferior and medial as it transitions toward the rectum. Fine vasa recta pass from the left colic branches to the colonic wall, where the haustra, taeniae coli, and appendices epiploicae mark the external surface. Accompanying veins run parallel in the mesentery. For left-sided colonic surgery, this arterial anatomy determines what stays perfused after a high or low ligation of the inferior mesenteric artery. The ascending branch of the left colic artery contributes to the marginal artery near the splenic flexure (Griffiths point), a watershed region that is prone to ischemic colitis during hypotension or after vascular ligation. This view also supports teaching of collateral pathways between superior mesenteric and inferior mesenteric territories, and why inadequate marginal flow can complicate anastomotic healing after a left hemicolectomy or sigmoid resection. Use it in gross anatomy and GI surgery modules when you need a clean correlation between mesocolic vessel geometry and the surface anatomy of the colon.
