Description
Anteriorly, the distal rectum and anal canal sit midline within the lesser pelvis, invested by an interwoven rectal plexus that blends autonomic fibers around the rectal wall. Superiorly, lumbar and sacral spinal nerve roots descend toward the sacral promontory and anterior sacrum, while the pelvic splanchnic nerves (S2 to S4) course anteromedially to join the inferior hypogastric plexus before distributing to the rectum. Laterally, the pelvic sidewall and ilia frame the field, with sympathetic trunks and segmental ganglia running longitudinally beside the vertebral bodies and sacrum, sending sympathetic contributions via hypogastric nerves into the pelvic plexus. Color coding distinguishes somatic sacral plexus branches destined for the lower limb from the finer autonomic meshwork surrounding the hindgut. Rectal plexus anatomy matters because continence and defecation depend on intact parasympathetic outflow for rectal motility and internal anal sphincter relaxation, balanced against sympathetic tone and somatic control via the pudendal nerve. Small mistakes during total mesorectal excision for rectal cancer, low anterior resection, or abdominoperineal resection can injure the hypogastric nerves or pelvic plexus, leading to postoperative constipation, fecal incontinence, urinary dysfunction, and sexual dysfunction in men. This perspective also clarifies why sacral root pathology, pelvic fracture, or presacral dissection can produce mixed sensory, motor, and autonomic symptoms. A dense crossroads.
