Description
Across the anterior surface of the abdominal aorta, a dense autonomic web forms the aortic plexus, with sympathetic fibers from the thoracic and lumbar splanchnic nerves interlacing with parasympathetic contributions from the vagal trunks. Branches track inferiorly toward the aortic bifurcation and spread laterally along the origins of the renal, superior mesenteric, and inferior mesenteric arteries, where they blend with the renal, superior mesenteric, and inferior mesenteric plexuses. Framing the field, the large intestine encircles the central abdomen, with small-intestinal loops occupying the midline and lower quadrants anterior to the great vessels. Anterior exposure matters because the aortic plexus is a surgical neighbor, not an abstract network, and its fibers concentrate around the prevertebral ganglia and arterial takeoffs that surgeons handle directly. Injury or excessive traction on these sympathetic pathways during open aortic repair, retroperitoneal lymph node dissection, or anterior lumbar approaches can contribute to postoperative autonomic dysfunction, including ejaculatory disorders in male patients.
