Description
Arising from the ulnar nerve in the distal forearm, the superficial branch courses anteriorly toward the ulnar side of the wrist, passing superficial to the flexor retinaculum to enter the palmar compartment. Distal to the pisiform and the hook of hamate, its terminal filaments spread across the hypothenar region and divide into common and proper palmar digital nerves supplying cutaneous innervation to the little finger and the ulnar half of the ring finger, extending into the digital pads. Adjacent landmarks typically include the ulnar artery and its superficial palmar branch, the palmaris brevis overlying the hypothenar eminence, and the flexor tendons deep to the palmar aponeurosis. Fine sensory twigs dominate. This anterior perspective matters because it tracks the sensory pathway most often implicated in ulnar tunnel (Guyon canal) syndromes, where a lesion can selectively affect the superficial branch and spare deep motor function to the interossei and adductor pollicis. Compression from a ganglion cyst, repetitive handlebar pressure, or a hook of hamate fracture can produce numbness and dysesthesia in the ulnar digits without the classic clawing seen with proximal ulnar neuropathy. Surgeons planning a volar approach to the ulnar wrist, and clinicians performing sensory mapping, rely on these distal branching patterns to localize injury.
