Arcing across the deep palmar surface, the deep branch of the ulnar nerve (ramus profundus nervi ulnaris) is traced after it passes through Guyon canal and turns laterally, deep to the flexor tendons and palmar aponeurosis. From an anterior perspective, it courses in close company with the deep palmar arterial arch, then fans into motor branches to the hypothenar muscles, all interossei, the third and fourth lumbricals, and adductor pollicis, with the carpal bones and metacarpal bases forming the posterior osseous backdrop. The hook of the hamate and pisiform sit ulnarly as key proximal landmarks, while the metacarpophalangeal joints and proximal phalanges lie distal to the nerve’s terminal distribution. Small accompanying vessels and superficial venous channels remain visible anteriorly, but the nerve is clearly positioned in the deep plane. Clinically, this view matters because ulnar neuropathy at the wrist often spares dorsal ulnar cutaneous sensation yet produces intrinsic hand weakness, and the deep branch is the primary motor limb at risk. Compression or injury within Guyon canal, classically from a ganglion cyst, handlebar palsy, or fracture of the hamate hook, can selectively affect the deep branch and present with weak finger abduction and adduction and impaired pinch from adductor pollicis. Clear separation from the superficial branch helps teach why sensory findings can be minimal despite marked motor deficit. Hand surgery and peripheral nerve modules use this anatomy to plan decompression of ulnar tunnel and to localize lesions based on motor patterns in interossei versus hypothenar involvement. It also fits atlases and journal figures discussing ulnar tunnel syndrome, hamate hook pathology, and the relationship of the deep ulnar nerve to the deep palmar arch during volar approaches.
