Description
Running inferior to the zygomatic arch, the marginal mandibular branch of the facial nerve (ramus marginalis mandibulae nervi facialis) is traced as it leaves the parotid plexus and courses anteroinferiorly toward the mandibular body and angle. From an anterior perspective, its slender fibers lie superficial to the masseter and platysma as they approach the lower lip, with the mandible and dentition providing hard-tissue landmarks for orientation. Medially, the mental region and lower incisor-canine arcade frame the expected territory of the depressor anguli oris and depressor labii inferioris. Fine cervical and facial nerve twigs may be suggested as part of the broader cervicofacial network. This is the branch surgeons worry about during submandibular, parotid, and neck dissections. The marginal mandibular nerve often runs close to the inferior border of the mandible and can dip 1 to 2 cm below it near the angle, which explains its vulnerability in a submandibular incision or during elevation of a platysma flap. Injury produces an asymmetric smile with lower lip incompetence, a pattern that helps distinguish a peripheral branch lesion from central facial palsy that typically spares the forehead. Its relationship to the facial artery and vein as they cross the mandible is also a practical landmark when you need a quick mental map in the operating room.
