Description
Arising as the larger terminal branch of the brachial artery in the cubital fossa, the ulnar artery descends in the anterior compartment of the forearm deep to the pronator teres and flexor digitorum superficialis, then becomes more superficial in the distal third as it courses lateral to the flexor carpi ulnaris tendon. Distally it passes superficial to the flexor retinaculum through Guyon’s canal, medial to the hook of hamate, accompanied by the ulnar nerve, before contributing predominantly to the superficial palmar arch. Digital branches radiate distally toward the fingers, with the deep palmar branch turning laterally and posteriorly to participate in the deep palmar arch. Small-caliber arterioles are suggested in the palmar tissues. Landmarks matter. An anterior perspective like this is the one you want when teaching or planning around ulnar-sided wrist access and injury patterns. The relationship of the ulnar artery to the pisiform, hook of hamate, and ulnar nerve explains why ganglion cysts, hook of hamate fractures, and repetitive hypothenar trauma can produce ischemic symptoms, including hypothenar hammer syndrome with distal embolization to the common and proper palmar digital arteries. It also frames the Allen test conceptually by clarifying how much of the superficial palmar arch depends on ulnar inflow and where collateral supply from the radial artery may (or may not) be reliable. Educators can drop this plate directly into upper limb anatomy teaching on the forearm and hand, vascular anatomy modules, or OSCE prep for pulse palpation at the ulnar aspect of the wrist and neurovascular assessment after trauma.
