Description
Running superficially across the anterior cheek, the zygomaticus minor arises from the zygomatic bone just lateral to the infraorbital margin and descends inferomedially toward the upper lip, blending with the orbicularis oris and fibers of the levator labii superioris. Lateral and slightly inferior to it, the zygomaticus major courses from the zygomatic bone to the modiolus at the angle of the mouth, creating a clear paired set of oblique muscle bands in an anterior perspective. Encircling the palpebral fissure, the orbicularis oculi sits superior to both zygomatic muscles, while the masseter occupies the lateral mandibular ramus posterior to the cheek musculature. Fiber direction is explicit. This anterior, skin-removed view matters because the zygomaticus minor lies in the same superficial plane that cosmetic and reconstructive surgeons traverse during midface lifts, filler placement, and malar dissection, where preserving facial nerve branches (zygomatic and buccal) and avoiding asymmetric upper lip elevation are daily concerns. The relationship between zygomaticus minor, levator labii superioris, and the nasolabial fold helps explain why weakness after iatrogenic nerve injury can flatten the fold and reduce upper lip elevation, while hyperactivity can accentuate it. For facial expression teaching, the contrast between zygomaticus major (smile commissure retraction) and zygomaticus minor (upper lip elevation) is immediate.
