Description
Anteriorly, the axis (C2, epistropheus) is dominated by the dens (odontoid process) projecting superiorly from the vertebral body at the midline. Flanking the body, the superior articular facets sit superolaterally and angle to meet the inferior articular facets of the atlas (C1), while the transverse processes extend laterally toward the transverse foramina. Inferior to the dens, the vertebral body broadens, and the uncinate margins begin to define the cervical uncovertebral region. Small details matter here. Clinically, the odontoid is a hinge point for atlantoaxial rotation and a frequent site of fracture, with Anderson and D'Alonzo type II fractures at the base of the dens carrying a higher risk of nonunion. An anterior perspective clarifies the relationship between the dens and the C2 body, which helps when teaching stability concepts around the transverse ligament of the atlas and when planning screw trajectories for anterior odontoid fixation or assessing alignment after trauma.
