Description
Viewed from the lateral aspect, the axis (C2, epistropheus) is presented with the dens (odontoid process) projecting superiorly from the vertebral body toward the anterior arch of C1. Posterior to the body, the laminae converge into a stout, commonly bifid spinous process, while the superior and inferior articular processes sit at the posterior corners of the vertebral column, forming the C1 to C2 and C2 to C3 zygapophysial joints. The transverse process and transverse foramen lie lateral to the vertebral body, and the pedicle and pars interarticularis bridge the body to the posterior elements. Relationships read cleanly in profile. Rotation at the atlanto-axial complex centers on the dens, so this lateral perspective helps when teaching why disruption of the transverse ligament or a dens fracture can destabilize the craniovertebral junction. Type II odontoid fractures are a common adult injury after a fall, and they are clinically problematic because of nonunion risk and proximity to the spinal cord and vertebral arteries. Posterior element anatomy matters too, since C2 pars or pedicle instrumentation depends on appreciating the lateral mass, the trajectory toward the body, and the nearby transverse foramen.
