Description
Arcing between the anterior margin of the acromion and the lateral border of the coracoid process, the coracoacromial ligament forms a broad fibrous band spanning the superior aspect of the glenohumeral joint. Inferior to this arch sit the humeral head and greater tubercle, capped by articular cartilage, with the glenoid cavity of the scapula positioned medially and slightly posterior. The distal clavicle lies superior and medial to the acromion at the acromioclavicular joint, while a rotator cuff element, consistent with the supraspinatus tendon, courses laterally beneath the ligament toward the greater tubercle. Part of the thoracic cage appears posteromedial as an orientation landmark. Clinically, the coracoacromial ligament defines the roof of the subacromial space and becomes central when teaching or planning management of subacromial impingement and rotator cuff disease, where repeated contact between the coracoacromial arch and the supraspinatus tendon or subacromial bursa contributes to pain with elevation. An anterior perspective clarifies how acromial morphology and ligament thickness can narrow this interval, and it aligns with the bursal-sided view encountered during arthroscopy. A tight, ossified, or spur-associated arch is a common anatomic substrate in outlet-type impingement.
