Description
Spanning the superior aspect of the shoulder, the coracoacromial ligament runs from the lateral border of the coracoid process to the anterior undersurface of the acromion, forming a stout band that bridges the scapula over the humeral head. Deep to it, the greater tubercle and superior articular surface of the humerus sit inferiorly within the glenohumeral joint, while the clavicle meets the acromion medially at the acromioclavicular joint. The coracoacromial arch is the landmark here. Dense, parallel fibers emphasize its ligamentous character rather than tendon. Clinical relevance centers on subacromial space mechanics: the coracoacromial ligament forms the roof of the subacromial bursa and the supraspinatus outlet, so thickening, traction spurs at its acromial attachment, or an anteroinferiorly curved acromion can contribute to external impingement and rotator cuff tendinopathy. Surgeons also treat it with respect during arthroscopic subacromial decompression and acromioplasty, where partial release may improve clearance but complete division can permit anterosuperior escape in massive cuff tears. Its relationship to the long head of the biceps tendon and the rotator interval is often discussed, but the arch itself remains the fixed constraint.
