Description
Anterior to the shoulder girdle, the distal clavicle meets the acromion of the scapula to form the acromioclavicular joint, reinforced superiorly by the acromioclavicular ligament spanning the joint line. The ligament sits superficial to the joint capsule and lies superior and slightly posterior to the articular surfaces, bridging the lateral end of the clavicle to the acromial facet. Inferior to this complex, the proximal humerus aligns with the glenoid region, while the upper ribs provide a thoracic reference medial and inferior to the joint. Orientation at the acromioclavicular joint matters in trauma and reconstruction. A direct blow to the lateral shoulder commonly sprains or ruptures the acromioclavicular ligament, and when the coracoclavicular ligaments fail as well, the distal clavicle displaces superiorly, producing the classic step-off deformity seen in higher-grade AC separations (Rockwood classification). This anterior view clarifies why palpation pain localizes to the joint line and why surgical stabilization targets both horizontal stability (AC ligament/capsule) and vertical stability (coracoclavicular complex).
