Description
Spanning from the anterior superior iliac spine to the pubic tubercle, the inguinal ligament forms the inferior rolled edge of the external oblique aponeurosis along the anterior pelvis of an adult male. Medially, its attachment blends with the lacunar ligament at the pubic crest and helps define the medial boundary of the femoral ring, while laterally it lies superficial to the iliopsoas compartment as the femoral vessels pass deep to it. Superior to the ligament, the aponeurotic fibers of external oblique contribute to the anterior wall of the inguinal canal; inferior to it, the fascia lata and the iliopubic tract reinforce the transition into the upper thigh. Clear landmarks. Anterior orientation matters here because most clinically relevant pathology is approached from the front: the inguinal ligament is the reference line for distinguishing inguinal from femoral hernias and for describing lymph node basins in the groin. Surgeons and trainees use this surface anatomy to anticipate the course of the spermatic cord through the superficial inguinal ring and to avoid iatrogenic injury to the ilioinguinal and genital branch of the genitofemoral nerve during open inguinal hernia repair. The same relationships guide safe femoral arterial puncture, which should be performed inferior to the inguinal ligament to reduce the risk of retroperitoneal hemorrhage from a high stick.
