Description
Running from the anterior superior iliac spine to the pubic tubercle, the inguinal ligament forms a taut band along the groin crease, seen here in lateral profile against the anteroinferior margin of the abdominal wall. Superior to it lie the aponeurotic fibers of the external oblique and adjacent fascia, while inferiorly the iliopsoas region transitions toward the proximal femur and the anterior hip capsule. Posterior and superior in the same field, the iliac wing and lower lumbar vertebrae provide bony context, with the sacroiliac region receding dorsally. The acetabulum and femoral head sit lateral to the pubic ramus, clarifying how close the ligament runs to the anterior rim of the hip joint. Clinically, this angle is the one you want when teaching the myopectineal orifice and the mechanics of groin hernias. The inguinal ligament forms the inferior border of the inguinal canal and the superior boundary of the femoral triangle, so its position helps distinguish indirect and direct inguinal hernia trajectories from a femoral hernia that passes inferior to the ligament and may compress the femoral vein. It is also a dependable surface landmark for the femoral pulse and for femoral arterial access, where misidentifying the ligament can shift puncture level and raise the risk of retroperitoneal hemorrhage. Surgical educators can pair this view with open inguinal hernia repair steps (Lichtenstein mesh placement and suture bites along the inguinal ligament) and with regional anesthesia teaching for fascia iliaca and femoral nerve blocks, where the ligament marks the proximal boundary of needle approach.
