Description
Presented in sagittal section with an anterior orientation, the male heart is opened to expose the left atrium superior to the left ventricle and the mitral (bicuspid) valve spanning the atrioventricular junction. Two mitral valve leaflets (anterior and posterior cusps) attach along the fibrous annulus, with chordae tendineae coursing inferiorly to the anterolateral and posteromedial papillary muscles on the ventricular wall. Adjacent structures in the same cut include the thicker left ventricular myocardium compared with the right ventricle, the right atrium and right ventricle on the contralateral side of the section, and portions of the proximal aorta and pulmonary trunk superiorly. An anterior view through a sagittal cut offers a clean read on subvalvular anatomy, the spatial relationship that drives most mitral regurgitation mechanics: leaflet coaptation in systole depends on intact chordae and papillary muscle position within the left ventricle. This is the anatomy behind acute severe MR after posteromedial papillary muscle rupture in inferior myocardial infarction, and behind flail leaflet when chordae tendineae rupture. It also maps directly onto mitral repair concepts, including annuloplasty sizing and chordal replacement, where millimeters matter. Use this rendering in gross anatomy and cardiovascular physiology teaching to anchor the difference between right and left ventricular wall thickness while keeping attention on the bicuspid valve apparatus, and in cardiology or cardiothoracic surgery materials to explain Carpentier mechanisms of MR, leaflet scallops, and the targets of valve-sparing repair.
