Description
Radiating from the ophthalmic artery within the orbit, the muscular branches course anteriorly and laterally toward the extraocular muscles, running in close company with the superior ophthalmic vein and smaller orbital venous channels. Superiorly, vessels track toward the levator palpebrae superioris and superior rectus region beneath the orbital roof, while inferior branches descend toward the inferior rectus and inferior oblique along the orbital floor. Medially, fine arterial twigs pass near the lacrimal sac area and anterior ethmoidal region, while more lateral channels approach the lacrimal gland in the superolateral orbit. Portions of the maxilla and zygomatic bone frame the anterior orbital aperture. Arterial supply to the extraocular musculature matters because these vessels sit at the crossroads of ophthalmic surgery, trauma, and orbital inflammation, where small changes in flow can produce outsized functional deficits. Injury near the orbital apex can compromise proximal ophthalmic artery branches at the same time the oculomotor, trochlear, and abducens nerves are at risk, a pattern that helps explain combined ophthalmoplegia after blowout fractures or compressive lesions. This view also helps teach why carotid cavernous fistula and cavernous sinus thrombosis can present with orbital congestion, given the tight arterial and venous packing around the extraocular motor apparatus.
